ABSTRACT

Objective:To evaluate catheter directed thrombolysis in the treatment of early acute superior mesenteric artery embolization.Methods:The clinical data of 19 cases of acute superior mesenteric artery embolization treated with catheter directed thrombolysis therapy in the First Affiliated Hospital of Shandong First Medical University from Aug 2015 to Jul 2020 was retrospectively analyzed.Results:Fifteen cases were treated successfully with catheter directed thrombolysis , the blood flow and intestinal function restored significantly. The condition of 4 patients worsened during catheter directed thrombolysis therapy. Among them, three cases of intestinal necrosis were later confirmed by exploratory laparotomy and they recovered well after enterectomy. One elderly patient unsuitable for laparotomy died.Conclusion:Catheter directed thrombolysis is effective in the treatment of acute superior mesenteric artery embolization in early stage without intestinal necrosis.

ABSTRACT

Objective:To compare the short-and long-term effects of conservative and interventional treatment in patients with lower extremity arteriosclerosis obliterans of Fontaine stage Ⅱ.Methods:This is a retrospective analysis,184 patients (219 limbs) admitted to the Department of Vascular Surgery, First Affiliated Hospital of Shandong First Medical University from Jun 2014 to Jun 2017, were divided into the conservative treatment group (93 limbs) and interventional treatment group (126 limbs).Results:The short-term effective rate in the intervention group was better than that of the conservative group (83.33% vs. 45.16%, P<0.05). After one year follow-up, the total effective rate in the groups was similar (60.32% vs. 53.76%, P>0.05). While the total effective rate of the conservative group was better than that of the intervention group (60.21% vs. 53.17%) after the 3-year follow-up,though the differences were not statistically significant ( P>0.05). The rates of all-cause death, amputation, and cardiovascular events in the conservative group were lower; though the difference was not significant ( P>0.05). However, the intervention group had a significantly higher rate of restenosis and re-intervention rate during the 3-year follow-up period ( P<0.05). Conclusion:Compared to intervention group,the conservative treatment is more economical ,effective and safe for patients with lower extremity arteriosclerosis obliterans of Fontaine stage Ⅱ.

ABSTRACT

Exosomes are cell-derived vesicles and have biological activity.The diameter of exosomes is between 30 and 120 nm.Exosomes participate in the invasion,metastasis and multi-drug resistance of malignant tumors.In the field of radiotherapy,it has been proven that radiation-induced changes in the secretion of exosomes from tumor cells can affect intercellular communication and enhance the radiotherapy resistance of tumor cells.In this article,the research progress on exosomes in the radiotherapy for malignant tumors was reviewed.

ABSTRACT

Objective:To evaluate the clinical outcomes of patients after intensity modulated radiation therapy (IMRT) for hepatocellu-lar carcinoma (HCC) with portal vein tumor thrombosis (PVTT) or inferior vena cava tumor thrombosis (IVTT). Methods:A registry data-base of 41 patients treated with IMRT for HCC with PVTT or IVTT in Radiotherapy Center of No.3 Hospital of Beijing Chinese People Armed Police Forces between January 2013 and May 2015 were analyzed retrospectively. The biological effective dose of radiation was ranged 48-94Gy. Short-term effects and prospective efficacy were observed. Log-rank test, Cox regression were used for survival analysis and prognostic factor identification. Results: Grade 2 or higher level gastrointestinal reaction was observed in 12 patients (29.3%). Hematological toxicity was observed in 11 patients (26.8%), and radiation-induced liver disease was observed in 6 patients (14.6%). We found that 29.3%of patients achieved a complete response and 31.7%of patients achieved a partial response. The stable and progressive disease rates were 39.0%and 0.0%, respectively. Median patient survival was 11 months, and the 1-year survival rate was 34.1%. The significant independent variables associated with the overall survival included Child-Pugh grade, AFP test, and CHE lev-el. Conclusion:IMRT is a safe and effective treatment for PVTT or IVTT in patients with HCC.

ABSTRACT

Objective:To observe the effects of 8-week interval anaerobic training on the respiratory function, airway inflammatoion and exercise performance in the asthmatic children,and to provide theoretic and practical evidence for special exercise prescription for the asthmatic children.Methods:Forty-six mild asthmatic children were willingly divided into exercise group (n=23)and control group (n=23).The children in exercise group conducted 8 weeks interval anaerobic training while the children in control group maintained intrinsical lifestyle.Before and after experiment,the respiratory function,airway inflammatory reaction and exercise performance were determined by pneumonometer,nitric oxide determinator and cycle ergometer.Results:One patient of each group was lost to follow up.509 persons in exercise group finished the training;11 persons had adverse reaction and the incidence was 2.2%.After experiment,compared with before experiment,the peak expiratory flow (PEF)in exercise group after experiment was increased (P 0.05).Compared with control group after experiment,all indexes above (PEF,FeNO,MP and PP)in exercise group existed statistical differences (P <0.05).Conclusion:Interval anaerobic training has safetiveness and effectiveness under proper supervision and could improve the respiratory function,reduce the airway inflammation and enhance the exercise performance in the mild asthmatic children.

ABSTRACT

Objective:To explore the inflammatory responses of macrophages treated with Helicobacter pylori. Methods:Cytokines IL-23,IL-10,TNF-α and IL-8 in cell culture supernatant of macrophages stimulated with Helicobacter pylori were determined by ELISA kits,and the expression of intracellular proteins NOS2 and COX2 in Helicobacter pylori treated macrophages was analyzed by Western blot. Then,the apoptosis of Helicobacter pylori stimulated macrophages was detected by flow cytometry. Results:The secretion of cytokines IL-23,IL-10,TNF-α and IL-8 in the culture supernatant of Helicobacter pylori treated macrophages increased significantly (P<0. 05),and the expression of NOS2 and COX2 was enhanced evidently(P<0. 05). Meanwhile,helicobacter pylori could induce the apoptosis of macrophages obviously ( P<0. 03 ) . Conclusion: The inflammatory responses of macrophages treated with Helicobacter pylori would be promoted to inhibit or kill Helicobacter pylori,furthermore,Helicobacter pylori could induce the apotosis of macropha-ges.

ABSTRACT

Objective To explore the influence of 8-week aerobic exercise in heart rate variability (HRV)in obese adolescents and to provide the evidence for formulating special exercise prescription.Methods Forty male obese adolescents (BMI≥25 kg·m-2 )were randomly divided into exercise group (n=20)and control group (n=20). The subjects in exercise group conducted a 8-week (five times/week,50-60 min)aerobic exercise while those in control group maintained their routine lifestyle. The body composition, hemodynamic parameters including heart rate(HR)and blood pressure and HRV parameters including total power (TP),normalized low frequency (LFn), normalized high frequency (HFn)and LFn/HFn ratio were measured before and after experiment.Results There were no significant differences of the indicators of the subjects between two groups before experiment (P>0.05). At the end of experiment,compared before experiment,the fat mass(FM),the percentage of body fat(BF%), the rest HR,SBP and DBP were decreased (P0.05).Compared control group,the FM,the BF%, the rest HR,SBP,DBP,LFn and LFn/HFn ratio were decreased while the TP and HFn were increased (P<0.01)in exercise group after experiment.Conclusion Aerobic exercise may effectively reduce the sympathetic activity and increase the vagal tone in the obese adolescents.

ABSTRACT

Esthesioneuroblastoma is one of the most uncommon nasal malignancies which is characterized by low incidence and high misdiagnosis rate.The only prognosis factor is the metastasis on lymph node of neck.Its clinical symptoms are associated with tumor infiltrating extent.Pathological diagnosis is the “gold standard” on esthesioneuroblastoma,and the Kadish staging based on the image is the most important staging standard.The traditional operation therapy mode has been replaced by the combined method of operation and radiotherapy,with the adjunctive therapy of chemotherapy.

ABSTRACT

Objectives To evaluate modified transthoracic esophageal transection (modified Walker's procedure) for management of postoperative rebleeding of portal hypertension.Methods Since 1983,we adopted modified Walker's operation to make low esophageal transection and transect intraabdominal recurrent varix of the coronary vein in the treatment of esophageal variceal rebleeding in portal hypertensive patients who had received previous surgeries aiming at portal hypertension.Results In 53 cases of postoperative rebleeding of portal hypertension,49 cases underwent elective operation with no mortality,4 cases received emergency surgery,one died post-operatively due to hepatic failure.The postoperative control rate of recurrent bleeding was 100%,and the overall postoperative mortality was 2%.The longest postoperative survival time was 24 years,and the 5-year survival rate was 77%.There were no postoperative recurrent hemorrhage and hepatic encephalopathy.Conclusions Modified transthoracic esophageal transection (modified Walker's procedure) is easy to perform,good at saving operative time,with a reliable immediate hemostatic effect and long-term control of variceal hemorrhage,hence is a effective remedy operative method for postoperative rebleeding of portal hypertensive patients after failed previous operations.

ABSTRACT

Conformal radiotherapy is an important therapeutic strategy for primary hepatic carcinoma,which has high local control rate.However,the frequency of intra- and extra-hepatic recurrences is very high.Sorafenib is the only drug that has been demonstrated effectiveness to increase overall survival in advanced or metastatic hepatocellular carcinoma.The combination about radiotherapy and sorafenib can increase the radiosensitiveness,and inhibit the angiogenesis,and relieve the hypoxia,as well as synchronize the cell cycle.A lot of biological studies of sorafenib combined with radiotherapy have been camied out,and the phase Ⅰ -Ⅱ clinical trials are is in progress.

ABSTRACT

Objective To investigate the relationship between the 6-Keto-PGF1α,TXB2 and the endotheliosis and thrombosis of the new endomembrance on the prostheses seeded by CD34 stem cells.Method Twelve dogs were evenly divided into two groups; ePTFE prostheses and weaved Dacron prostheses implanted into the abdominal aorta(AA) and inferior vena cava (IVC).in each group, 4 dogs were implanted by prostheses seeded by CD34+ stem cells, 2 dogs were implanted prostheses treated by autogenous blood only as control.The peripheral blood was collected in preoperative and 3 ,7,14,30,60 days postoperation.light and electron microscopy were applied to observe the endotheliosis and thrombosis of the new endomembrance when harvesting all the prostheses in the 60 days.Result The platelet, 6-Keto-PG F1α,TXB2 and P/T in experimental group was significantly different compared with control group.The new endomembrane in control group was significantly thickener than the experimental group.Conclusions CD34+ stem cell inhibits PGF1α and P/T ratio decrease, restrains TXB2 and platelet excessively rise, prevents intimal hyperplasia and thrombosis in a CD34+ stem cells seeded blood vessel prostheses.

ABSTRACT

Objective: To discuss the feasibility of immune tolerance in rat small bowel trans-plantation induced by immature dendritic cells isolated from SD and Wistar rat. Methods: Dendritic cells isolated from liver or bone marrow of inbred strain Wistar and SD rat were cultured for five to nine days. A week before operation, the recipient rats were injected 2?106 inmature DCs from vena dorsalis penis and observed. The models of heterotopic small bowel transplantation (SBT: SD→ Wistar)were established. Acute immuno- rejection levels were evaluated by HE. Results: Varying degrees of rejection were observed in 3, 5, 7 days after operation. Conclusion: Immature DC can induce allogenic immunological tolerance of rat small bowel transplantation.
